I was reading about the 787 that nose dived, apparently the stewardess accidentally pressed the seat forward button, which sent the captain forward, squashing him into the controls, de-activating the auto pilot with a full nose down.

50 passengers smashed into the ceiling then ontop of the seated passengers when they pulled the nose back up.
