I believe they are using that term in the same fashion the Bill Clinton was often referred to as "the First Black President".

You understand, they aren't trying to out Obama or anything, just say that he has a general record of supporting Homosexual rights.

Hence his response to Russia right now being of particular interest. Does he stand his ground and call them on what, to him and his followers, must seem like a horrible thing? Or does he stand back and do nothing, acting as if the issue doesn't matter that much, to keep the peace overall.

My guess, based on nothing but a feeling is that he will quietly release a press message that few will hear, and then act as if nothing involving gays has ever happened in Russia at all.

It's what governments all do, most of the time, on issues that they consider third tier or below.
