In the case of Seffner, they're created with a backhoe under cover of darkness.

I'm thinking no rebar and 3 or 2 sack cement and maybe a thinner slab.

Notice the cracks in the soil running along the streets?

Almost like there was a massive cavern deep in there and the construction making the streets cracked the layer above.

I suppose the crew wouldn't know if they made a HUGE crack from a jack hammer. It doesn't take too much to start a crack as they spread on their own.

Or it cracked because it didn't get any rain water after the road was made. Dried it up and cracked.

In fact, most sink holes are thought to be created by water leaving the area. Preventing water penetration from construction is a good way to do that.
