Yep, seen it once myself, I just missed it.

I suspect wrong size tires/rims on the truck which led to the wheel coming off. Possibly even the wrong bolt pattern with an adapter.

Yep, bet he will be off the roads for a bit. I was driving down a hwy and it is amazing how sometimes you have an intuition. I was following a truck but was back enough and I got that tingly feeling... so I was checking the other lanes for an exit. Thanking my luck stars someone was looking out for us as a shovel came straight at us like a missile. It was the strangest thing. Thankfully I was able to veer to the right for it just to miss us.
