He could've splipped and fell and that's how his pecker ended up inside that woman.
He has PTSD and can't control himself and kills his cellmate in a delusionary outburst.

The other guy could've been sleep walking dreaming he was at chuck e cheese playing that whack a mole game and it turned out it was the infant instead.

There is no accidently punching a baby to death.

People act like it's easy to have people killed in prison. It isn't, particularly. Anyone doing it will probably be caught, so they have to have a good reason and be willing to pay the price for it.

A random cellmate probably won't be a killer, or someone with nothing to lose. So it's harder than it seems for them to find the will to murder someone like that.

What a loss to the world.
