with electronic medical records, doctors order stuff on the wrong patients. This is one of the "tickets" we used to deal with...provider ordered wrong lab, procedure, nursing order on wrong patient, can it be fixed?

Also, many electronic medical records have so called automatic checks; if you don't have a DNR within so many hours of admission, the system will "flag" the doctor and recommend to put an order. We have had doctors click on the wrong button to exit the "warning" only to actually order it.

So to answer your question, many factors can happen, the most common one, doctor error, specially if they use electronic medical records and they start clicking all over to get their work done.
