Was it the Oxygen tank ?

What else would explode at a medical center?

Natural gas? Seems like they would detect that and isolate it in time.

Let me put this to bed right now. I'm a commercial HVAC guy. I've been in that place and its a dump. Super neglected. I'm right next door to the VA right now. It was a high pressure steam repair gone south. I know exactly which building its in and exactly what boilers they are. No conspiracy here. Contractor from another union company died when the pipe let go.

VA centers are run by mostly incompetent lazy bastards. EMS is what they call their janitors -Environmental Maintenance System. For most of them it's hard to think how they find their way to work each day.
