You're correct in stating that the pyramids were built as bunkers. The designers just didn't take fresh air into great enough consideration. Their ventilation systems were inadequate.

On the other hand, the plebs who fled to caves in higher ground were the ones who survived, having plenty of fresh air and water naturally stored up in deep caverns that went for miles and miles. They lived there for centuries, only leaving the caves to hunt. The ptb who took shelter in the pyramids all died.
