What is actually happening when a person hears music or voices "in their head"? Might there be a difference from "hearing" a song a person has already heard and "hearing" someone "speak" to them?

An adept, a group or an entity could be projecting thought-forms. You'd have to be dealing with a very focused group (with tech tools or biological understanding) or a very energetic individual. Entities do seldomly, and as a good rule of thumb are negative, very distinguishable by the content/qualia of the thoughts. The collective consciousness, your oversoul or your self would not pull yourself down.
If you can quiet your own mind, you can recognize any "voice in the silence".

An example of the focus needed, some decayed mystery schools practice to focus on an individual to make him/her believe he/her is having a heart attack or any other ailment, physical or mental, which in turns only attempt to hi-jack the individual's unaware chatter mind into harming him/herself.
It can go as complex as strong emotions, words, or anything for the narrative pushed. A bit of ongoing malicious focus and a bit manufacturing IRL and the individual if unaware of the source be push toward high levels of confusion.
