My honest opinion is that counter intelligence within N Korea would prevent any type of launch against the US. If their leadership is insane, the majority of the populace is not and even China realizes what would happen should N Korea launch against the USA. They know it would be better to punish their own child than to do nothing and allow a coalition to do so for them. It's politics.
