It will be interesting when the medical science leanrs to increase your expected life span more rapidly than what you actually age. At that point, the megarich who can afford constant medical upgrades and treatments will become essentially immortal.

If I had a family member that died in an accident and he or she had previously agreed to donate organs in such event.

Then to find out later that his or her heart was put into the chest of this satanic rat....that would piss me off.

An average person would not be able to get a heart transplant, not at his age. As for how many, he could have been having them since his late 80s because of his age, it would not do so well. Or, they can clone hearts, which would be kept secret.
You do remember someone close in the family died, was it his nephew or someone like that? Lord I hope his family have not been the donors, not necessarily by choice. That is a freaky thought. Really he must not believe in The Lord or he would be ready at his age to meet his maker. I guess his is another side than most.
